Mark Ouellette

Mark Ouellette, PT, MS, OCS, COMT

Mark is an Oregon state-licensed Physical Therapist and a board-certified Orthopedic Specialist by the American Board of Physical Therapy Specialties (ABPTS), as well as a Certified Orthopedic Manual Therapist (COMT) by the International Academy of Orthopedic Medicine (IAOM), the most advanced continuing education organization for diagnosis-specific manual therapy. Mark brings a unique perspective to his practice colored by his experience prior to entering the physical therapy profession.

Mark initially completed his undergraduate and graduate training at the University of Oregon within the Exercise and Movement Science Department, majoring in Biomechanics with minors in Sports Medicine and Motor Control. It is from this base, coupled with IAOM training, which yields his unique perspective of systematic method and inquiry, allowing him to obtain optimal results in the least amount of time.

Mark continues to pursue continuing education as he completes his Doctorate of Science with emphasis on Orthopedic Manual Physical Therapy at Texas Tech University Health Sciences.

Mark enjoys a variety of outdoor sports and enjoys playing music as well as martial arts. He is an advanced instructor in Seven Star Preying Mantis Kung-Fu and holds instructor rank in other disciplines.

Quinn Blanchard, DPT

Quinn Blanchard, DPT

Quinn is an Oregon state-licensed Physical Therapist who graduated in 2019 from Texas Tech University Health Sciences Center as a Doctor of Physical Therapy. He is a Certified Orthopedic Manual Therapist (COMT) by the International Academy of Orthopedic Medicine (IAOM), the most advanced continuing education organization for diagnosis-specific manual therapy. Quinn brings a unique perspective to his practice colored by his experience prior to entering thHe is currently expanding his knowledge base through courses offered by the International Academy of Orthopedic Medicine (IAOM), and has additionally found the experience of his colleagues here at Reclaim to be an invaluable resource. Quinn continues to study with the IAOM-US and looks forward to sharing his knowledge with his patients.

Kristi Hess PT, OCS

Kristi Hess PT, OCS

Kristi is an Oregon state-licensed Physical Therapist and a board certified Orthopedic Specialist by the American Board of Physical Therapy Specialties (ABPTS). She is also a Certified Orthopedic Manual Therapist (COMT) by the International Academy of Orthopedic Medicine (IAOM. She graduated from Indiana University with a Bachelor's Degree in Physical Therapy. In addition to IAOM-US coursework she has also taken extensive continuing education through the North American Institute of Orthopedic Physical Therapy (NAIOMPT) as well as the Institute of Physical Art (IPA), and is certified in Sportsmetrics, an ACL injury prevention program through Cincinnati Sports Medicine.

Kristi has also had training in vestibular and balance rehabilitation, for patients dealing with dizziness and/or falls. She has over 20 years of orthopedic physical therapy experience and enjoys working with patients of all ages to help restore function and get them back to work, recreation, and sports. Originally from Ohio, she has called Eugene home for the past 15+ years, where she enjoys cooking, yoga, stand up paddleboarding, and hiking.

Joel Thakkar, PT

Joel Thakkar, PT

Joel is an Oregon state-licensed Physical Therapist who graduated from the University of Queensland, in Brisbane, Australia as a Master of Physiotherapy. He is also a Certified Orthopedic Manual Therapist (COMT) by the International Academy of Orthopedic Medicine (IAOM.

Before becoming a Physio, Joel was an avid Rugby player and during his undergrad, worked as an Athletic Therapist with semi-professional Hockey players. This unique experience, as well as an inordinate love for sports lead him to develop a passion to better understand and treat concussions. To further this goal, he plans to seek accreditation through the CCMI (Complete Concussions Management Inc.) a world leader in concussion research and rehab, in addition to pursuing his COMT through the IAOM-Us.

In his "spare time" Joel enjoys all forms of exercise and outdoor activities, but most of all he loves spending time with his wife and son, whom they welcomed in May 2020. Joel is a life-long and diehard Oregon Ducks and Calgary Flames fan, and along with most everyone else in Eugene, a bandwagon LA Chargers fan. He loves to, and will, talk all things sports.

As a dual Canadian and American citizen, who roughed it with the Aussie's, Joel has experienced Physiotherapy in multiple countries and continents and brings a unique perspective to his patients. Joel strives to treat patients in a holistic, compassionate, and systematic way to help all people become the best versions of themselves.

Dan Lockman, PT, DPT

Dan Lockman, PT, DPT

Dan is an Oregon state-licensed Physical Therapist who graduated from the University of Montana as a Doctor of Physical Therapy. He is also a Certified Orthopedic Manual Therapist (COMT) by the International Academy of Orthopedic Medicine (IAOM.

Originally a Montana native, Dan is excited to explore all that Oregon has to offer and get engaged in the Eugene community. Dan enjoys working with a wide variety of orthopedic patients and enjoys partnering with his patients to form an individualized plan to achieve their goals and get back to doing what they love to do. In his free time, Dan enjoys hiking, backpacking, alpine skiing, paddle boarding, reading historical fiction novels, spending long hours looking at maps, and adventuring with his dog Samson.

Sean Jacob Magill, DPT, PT

Sean Jacob Magill, DPT, PT

Sean, an Oregon state-licensed Physical Therapist, earned his Doctor of Physical Therapy degree from the University of Southern California after following a rich foundation in Human Physiology from the University of Oregon. His core sciences background equips him with a strong understanding of human health and movement, which continues to be honed by ongoing advanced study through the International Academy of Orthopaedic Medicine (IAOM-US).

In his practice, Sean thoughtfully combines evidence-based neuromuscular retraining techniques to foster improvement in well-being and performance. Whether you're an aspiring athlete looking to improve your performance or seeking enhanced daily comfort, Sean's collaborative approach aims to help you tap into, and reach your body's unique potential.

Beyond his work, Sean's love for running and hiking with his dogs around Eugene's scenic trails helps him find solace and inspiration in the great outdoors. Simultaneously, he harbors a strong passion for the latest technological breakthroughs and is an avid sports fan, readily willing and wanting to discuss both.

Jim Quisenberry

Jim Quisenberry, PTA

Jim graduated from the Physical Therapist Assistant program at Lane Community College. He also has Bachelor of Science degrees in Civil Engineering and Construction Engineering Management from Oregon State University. Jim is also a member of the APTA/OPTA and is in the process of completing his Kinesio Taping certification training.

Jim enjoys working with patients of all ages, and has special interests in the spine, sports-related injuries and promoting lifelong health and wellness.

Born and raised in Salem, Jim, his wife and son, moved to Eugene 19 years ago. When not working, he enjoys hiking the forests, summiting mountains, backpacking, fishing, Beaver sports and daily walking. He also is a Referee for USA Swimming and a volunteer for OHSU’s ThinkFirst program which promotes bike helmet use/concussion awareness.

Mike Rhinesmith, PTA, LMT

Mike Rhinesmith, PTA, LMT

Mike graduated from the Physical Therapist Assistant Program at Chippewa Valley Technical College. He has spent most of his early career and clinical experience in an outpatient setting. In addition he has also been a Licensed Massage Therapist since 2014. He enjoys combining both therapeutic exercise as well as various manual techniques to best treat his patients. He has been training in IAOM specific courses and looking to specialize in Orthopedics.

Mike recently moved to Eugene from Milwaukee, WI trading in the Midwest for Pacific Northwest. In his free time, Mike enjoys basketball, exploring the outdoors, woodworking, and Packers football. He currently finds himself in the midst of a battle between good and evil trying to be converted to either a Ducks or Beavers fan.

Jessica Carlson, PTA

Jessica Carlson, PTA

Jessica Carlson is a Physical Therapist Assistant who graduated from Lane Community College in 2011. As a Certified Pilates Instructor since 2017, she integrates Stott Pilates basic principles within therapeutic exercises and activities of daily living to improve optimal spinal posture for proper loading of tissues and joints. She believes a combination of strengthening and lengthening tissues along with mindful movements is one of the many approaches for managing pain while enjoying a more functional lifestyle. She has previous experience in SNF, Home health, and Outpatient setting. She is continuing to expand her knowledge in manual therapy through International Academy Orthopedic Medicine (IAOM) with the goal of certification. We are pleased to have Jessica offer private Pilates instruction at Reclaim Physical Therapy as part of our Reclaim Wellness Program. Jessica enjoys pilates, hiking, running, concerts, vacation, and spending time with family.

Benjamin Garland, PTA

Benjamin Garland, PTA

Benjamin has been actively participating in the physical therapy profession since 2011. He holds both an Oregon and California state license as a Physical Therapist Assistant, earning his Associate of Applied Science degree from Ohlone College in Fremont California graduating with honors. He also has a Bachelor of Arts degree in Human Communications from California State University Monterey Bay where he graduated with distinction. During his time there he competed in both Cross Country running and track and field as the team captain. During his time competing as a runner for the university he placed 23rd at the 2014 NCAA Western Regional Cross Country Championships. When he was done competing at the collegiate level he placed 34th in the world at the International Spartan World Championships Obstacle Races in Squaw Valley in 2015. He has been a competitive rock climber, muay thai kickboxer, and is a a Brazilian jiu-jitsu practitioner. He once lived in South Lake Tahoe to pursue his love of snowboarding and skiing.

He believes caring for the body is an integral part of every person's well-being and it has been this belief and passion that has been the driving catalyst in his professional career. He likes to work hard and play hard. He continues to be a lifelong learner to hone his skills so that he can provide the highest quality of care to his patients. He relocated from his hometown of Santa Cruz California in 2023 to Eugene after falling in love with Oregon. Being an athlete is one of the "hats" he wears, the other is working for and being of service to others. He continues to be an athlete in his spare time and loves nothing more than to help other people with their bodies. With great enthusiasm, he looks forward to working with you at his new home, Reclaim Physical Therapy.

Jacob Foote PTA

Jacob Foote, PTA

Jake obtained his Associate in Applied Science as a Physical Therapist Assistant from Lane Community College and has most of his experience in the outpatient setting. He has clinical interest in orthopedics, neurology, and vestibular rehab. Jake has always enjoyed working with people. He grew up around Missoula, Montana where he received a Bachelor of Arts in Psychology from the University of Montana.

While in Missoula he worked in a traumatic brain injury department as a life skills trainer. He moved to Bozeman, Montana in 2014 and worked various jobs including as a certified nursing assistant at an assisted living facility. He moved to Eugene, Oregon in 2016 and worked as a qualified mental health associate for over 6 years. Outside of work he loves being in the outdoors and spending time with his wife and two children. He enjoys hiking, camping, snowboarding, playing disc golf, woodworking/carving, and playing hockey.
